Keep priorities, measurements, assessment questions and quote decisions in one place. Type your notes and print the pack or save it as a PDF.

This pack is free. Your entries stay on this page and are not saved automatically or submitted. Keep completed copies securely. It organises a discussion; it is not a safety assessment or an approved design.

1. Agree what matters most

Discuss this with the resident. Describe a task rather than assuming which equipment to buy.

2. Record useful measurements

Collect only what you can measure safely. State the units; a measurement alone does not establish suitability.

For room layouts, use the kitchen worktop and knee-space guide and bathroom turning and transfer guide to prepare questions for the project team.

3. Prepare the first professional conversation

“We would like help with this everyday task: ______. This is what currently happens: ______. What assessment or trial would help us choose the next step?”

  • Who assesses personal suitability, and who checks design and installation?
  • What can be demonstrated or trialled before we buy?
  • Which permissions or written funding decisions are needed first?
  • What is the backup arrangement while work or equipment is unavailable?

4. Compare the whole quote

Ask each contractor for the same access goal and an itemised written scope. Include design, approvals, preparation, materials, installation, taxes, disposal, exclusions, maintenance and aftercare. Keep a separate contingency.

Plan a home adaptation schedule

Use these original prompts to agree a sequence with the resident and project team. In the notes below, record a responsible person, target date and dependency for each stage. These are planning stages, not a promise that every project follows one timetable.

A discussion schedule from priorities to review
StageAgree before moving on
Priorities and assessmentThe task, preferences, assessor and any urgent interim arrangements.
Design, trials and cost evidenceThe proposed solution, measurements, equipment compatibility, alternatives and any quotes needed for a funding request.
Permissions and fundingOwner consent, required permits, the funding decision and any private costs.
Quotes and contractA comparable scope, exclusions, programme, payments and process for variations.
InstallationAccess and essential facilities during the work, inspections and who handles delays.
Handover and reviewRequired documents, instructions, the intended task check and follow-up contact.

Design, quotes and funding may need more than one round of discussion; cost evidence can be needed before a funding decision. Assessment queues, permissions and contractor availability can also have different timescales. Ask for each stage separately rather than treating a grant-decision period as a completion date. If access needs change or a hospital return is approaching, tell the assessment team promptly and discuss interim arrangements.

Use the assessment guide, country funding notes and hospital-to-home record alongside this schedule.

5. Agree the next step and review

Frequently asked questions

How do I create a home adaptation schedule?

Agree the priority task, assessment, design and cost evidence, permissions and funding, contract, installation and handover. Give each stage a responsible contact, target date and dependency, and record what happens if the plan changes. Use the schedule to organise decisions with the project team.

How long do home adaptations take?

There is no single worldwide completion time. Ask separately about assessment, design, permissions, funding decisions, procurement and installation. Request a project-specific programme and interim arrangements; an application-decision timeframe is not a promise that the building work will be completed by then.
